Acknowledgment of Risk: I acknowledge and understand that participation in events held through the 814 All Star Kids Club at the various locations involves certain inherent risks, including but not limited to physical injury. I fully accept and assume all risks associated with my participation in this program. Release of Liability: In consideration of being allowed to participate in the 814 All Star Kids Club. I hereby release, waive, discharge, and covenant not to sue any organizations associated with 814 All Star Kids Club, its affiliates, officers, directors, employees, volunteers, agents, and representatives (collectively, "Released Parties") from any and all claims, liabilities, damages, or expenses, including but not limited to personal injury, death, or property damage, arising out of or related to my participation in any of their events, whether caused by the negligence of the Released Parties or otherwise. Medical Consent: I consent to receive medical treatment that may be deemed necessary in the event of an injury or medical emergency. I agree to be responsible for any medical expenses incurred as a result of such treatment. Media Release: I grant permission to 814 All Star Kids Club to use my image, voice, or likeness in any photographs, videos, or other media taken during the event for promotional, educational, or other purposes without compensation. Understanding and Agreement: I have read this Liability Waiver and Release of Claims, understand its terms, and agree to be bound by its provisions. I am aware that by agreeing to this waiver below, I am giving up certain legal rights, including the right to sue the parties associated with the 814 All Star Kids Club.
